Directory Information Restriction Removal Request

Description

Removes the restriction of the release of directory information previously placed to have directory information withheld. This form will allow directory information to be released to the public.  "Directory information" is defined as the student's:

  • name
  • address (including e-mail address)
  • telephone number
  • date and place of birth
  • major
  • participation in officially recognized activities and sports
  • weight and height of members of athletic teams
  • dates of attendance
  • degrees and honors information
  • and the most recent previous educational agency or institution attended

A valid photo ID is required.
