INSTRUCTIONS: Complete this form and attach all appropriate documentation. This request form must be submitted no later than November 26th (Fall Commencement). If approved, the student can participate in commencement and their name will be called, but they will not receive their degree until all graduation requirements are met. Please note there is no guarantee that the student's name will be in the graduation program pending the timing of this request and the program printing date. Students requesting to participate in commencement must be within 6 credit hours of completing their degree.
